How to Refer a Family to the 100 Families Initiative

At the heart of the 100 Families Initiative is a simple promise: no family in our community should ever fall through the cracks. To make that promise a reality, we have created an easy, accessible referral system that ensures families in crisis receive timely support, coordinated care, and a compassionate advocate walking alongside them.

Families can now be referred in three convenient ways:

  1. Phone Call
    Community partners, agencies, churches, and even concerned neighbors can make a referral with just a phone call. To refer a family, please call 517-882-0056. Our team will gather the initial information needed and begin the process of connecting the family to resources and support.
  2. Email Referral
    Referrals can also be sent by email. This option is often helpful for agencies who already have basic information on hand and prefer to submit it in writing. You can email referrals to eleanorekue@hotmail.com. Once received, our team enters the referral into our system and routes it for immediate follow-up.
  3. HopeHub Referral (Our New Preferred Method)
    We are excited to share that agencies can now submit referrals directly through HopeHub, the secure digital platform powering our collaborative care model. After creating a free agency account, partners can complete a referral form that includes the family’s private information and pressing needs.
    Once submitted, the referral is automatically assigned to a trained Family Advocate, who will reach out to the family within 24 hours. This quick connection helps stabilize urgent situations and ensures families receive holistic, coordinated care without delay.

Our referral process is intentionally designed to be simple, fast, and accessible—because when a family is in crisis, time matters. Whether you are a community agency, a church, a school, or an individual who sees a family struggling, you can be part of the solution.
